Home care and home health care are two separate ways of helping aging individuals remain safely in the comfort of their homes. Home health care provides clinical services, while home care offers non-clinical help with daily living. In-home Care

Clear Path Home Care understands there is no place like your home and our agency has provided personal assistance services for our clients in their homes since 2013. Our compassionate caregivers and knowledgeable office staff offer assistance with activities of daily living to allow you or your loved one to age in place at home. However, if you or your loved one require or choose the levels of care listed below, our agency can supplement that care at the facility of your choice.

Independent Living

Typically, these residential communities offer an active lifestyle and freedom from the hassles of home maintenance for individuals age 55+. Landscaping, housekeeping, linen service, dining plans, transportation services and security are commonly offered. Amenities may also include fitness centers, pools, beauty and barber salons, woodworking shops, art studios or crafting rooms and entertainment spaces.

Assisted Living

Assisted living services are designed for individuals who value their independence but can still use a hand with everyday activities, such as meal preparation and assistance with bathing, dressing and medication management.

Skilled Nursing

Skilled nursing offers an advanced level of care and is appropriate for individuals who need post-surgical rehabilitation, wound care, cognitive and pain management or close medical supervision.

Memory Care

Memory care services are specifically designed for individuals living with Alzheimer’s and other forms of dementia. Services are designed to balance safety with engagement and quality of life.

Life Plan Communities

Life plan communities (also known as continuing care retirement communities) offer independent living options as well as assisted living, memory care and skilled nursing. This allows residents to move seamlessly from one level of care to another as needs change.

You may have patients who could benefit from home care services. This type of support may be particularly helpful following discharge from the hospital. Services like personal care, light housekeeping, companionship and medication reminders can go a long way toward keeping an individual safely and comfortably in their home.

We will be happy to meet with the client and family to discuss a care plan and determine if Clear Path Home Care is the best option for your patient.

Clear Path Home Care will work with you or your loved one to determine how many hours of assistance are required and what level of care is needed. You and our senior care relationship manager will create a care plan outlining all of our responsibilities with respect to the care of you or your loved one. 

Clear Path Home Care will also help you navigate submitting claims to your long-term care insurance provider or Life Insurance provider which clients routinely use to offset out of pocket expenses. 

Our agency has extensive resources in all the communities we serve to provide financial planning assistance, trusts, and financing your in-home care costs.

Yes, a long-term care insurance policy may cover the cost of your home care. All long-term care insurance policies are different and they cover various aspects of long-term care. Clear Path Home Care offers a free assessment of your policy to determine what is covered, how much the insurance company will pay for the care, and for how long. Additionally, once we have determined your long-term care policy’s benefits, Clear Path will engage directly with the insurance company to provide all the documentation needed to fulfill the policy’s requirements for as long as the policy continues to pay.

Life insurance policies also can offset home care costs. If you have a life insurance policy you may be able to access the cash value of the policy. Clear Path Home Care will work with you to determine if this option is the right one for you. Our agency also works closely with a legal expert team to help determine various ways to structure your assets to assist you in paying the costs of long-term care.
